This expansion brings a patient centric approach to healthcare, delivering hospital level care and comprehensive chronic disease management directly to patients’ homes.

• HiYH: Offers hospital-level care at home, reducing the need for hospital admissions and readmissions, thereby saving valuable bed space and healthcare costs.

• NovellaCare: Specializes in community based chronic disease management, providing personalized care plans to support patients in managing their health, promoting wellness, and preventing unnecessary hospitalizations.

Remote Patient Monitoring (RPM): A cornerstone of our services, RPM allows for health monitoring, early intervention, and improved health outcomes by addressing issues promptly within the comfort of the patient’s home.

Community Engagement and Well-Being:

• We are committed to enhancing the well-being of Savannah’s community, especially the senior population, through collaborations with local healthcare providers and organizations.

• Community Clinic Days: In partnership with senior centers, we will offer preventative screenings, wellness exams, and chronic disease management services. An initial Clinic Day Information Session is scheduled for February 5th at 10:30 am at the Moses Jackson Senior Center.

Local Roots, Strong Commitment: Co-founder Novella Tascoe, JD, MSHA, RRT and Chronic Care Director Allyson Blue, AGNP-C are both natives of Savannah. Their deep connections to the community fuel their enthusiasm for expanding these innovative models of care to Savannah.
